Hypothyroidism is a condition where there is decreased production of thyroid hormone by the thyroid gland. It is also known as “underactive thyroid”. It is an autoimmune disease. Thyroid hormones (T3, T4,TSH) are responsible for body metabolism. When thyroid glands doesn’t secrete sufficient amount of thyroid hormones then it causes slow metabolism of the body.
Hypothyroidism is mostly seen in women than men.
In children, it is known as cretinism. There is delay in the growth of the child.
Normal levels of thyroid hormones:
TSH: 0.5-5.5 mlU/L.
Total T3: 80-220 mlU/L
Total T4: 4.5-12.5 mlU/L
Free T3: 2.3-4.2 mlU/L
Free T4: 0.7-2.0 mlU/L
There is an axis which maintains normal range of thyroid hormone levels known as Hypothalamic- Pituitary-Thyroid Axis.
Hypothalamus sends signal to the anterior pituitary to release TSH and TSH stimulates thyroid gland to release Thyroxine (T4) hormone for normal functioning of many tissues of the body. T4 hormone thus gets converted to T3 hormone by certain enzymes in other organs of the body. Malfunction in this axis leads to the disorder. In hypothyroidism, there is an increased production of TSH and decreased production of thyroid hormones by thyroid gland due to many reasons/causes.
Complications of Hypothyroidism
Myxoedema coma- Untreated hypothyroidism leads to severe form known as myxoedema. There will be low blood pressure, low sugar level, more sweating and weakness.
Increased risk of cardiac disease.
Enlargement of the thyroid gland- There would be difficulty in swallowing food and breathing troubles.
Lathergy- Weakness.
Depression.
Infertility and abortions.
Causes of Hypothyroidism:
Iodine deficiency is the commonest cause.
Autoimmune thyroiditis (Hashimoto’s thyroiditis).
Dysfunction of the pituitary gland- In case of pituitary gland secreting decreased amount of TSH (thyroid stimulating hormone) Or dysfunction of the hypothalamus.
Any trauma or injury to the hypothalamus and pituitary gland.

USG of the neck and thyroid can be done to diagnose tumour or any other abnormality.
